HCV Patient Survey*: http://www.itd.depaul.edu/quickdata2/viewwebform.asp?id=2068 *The HCV Patient Survey is for individuals living with the Hepatitis C Virus (HCV).
HCV Caregiver Survey**: http://www.itd.depaul.edu/quickdata2/viewwebform.asp?id=2067 ** The HCV Caregiver Survey is for individuals who are caring for someone with HCV. A caregiver is defined as someone who attends to the needs of an individual with an illness.
Specific Instructions: Please fill out this anonymous survey that will take between 30-45 minutes to complete. All survey responses will be kept confidential – there will be no means by which survey responses can be linked to names. The survey will be administered via the Internet. Given the project's quickly approaching deadline, we ask that you fill out the survey within the next two weeks , if possible.
Patient vs. Caregiver Survey: Please note that there is a separate survey for caregivers of individuals with HCV, which uses a different link to the webpage. If you yourself are a caregiver of someone with HCV, please take the "HCV Caregiver Survey." If you are an individual with HCV, please take the "HCV Patient Survey," which will include directions on asking your primary caregiver (if you have one) to complete the Caregiver Survey.
